Cardiovascular Risk in Men Over 50: What the Standard Misses

Men in their fifties have a specific cardiovascular risk profile that standard primary care is not structured to assess adequately. The screening tools most commonly used were built to catch overt disease — elevated total cholesterol, hypertension at the threshold level, diabetes already declared on a fasting glucose. They were not built to identify the risk that accumulates for years before any of those numbers move.

Coronary artery calcium scoring gives a direct view of atherosclerotic plaque already present in the coronary arteries. It is a low-radiation CT scan, a strong predictor of cardiovascular events, and particularly useful for men in the intermediate risk category — where guidelines offer the least guidance and the most uncertainty. A zero CAC score in a 52-year-old man with borderline risk factors is clinically useful information. A CAC of 200 in that same man changes the treatment conversation entirely.
